End of the Line for Anthem Address Change

If you are an Indiana, Kentucky, Missouri, Ohio or Wisconsin provider and you are still sending paper claims or correspondence to Anthem Blue Cross Blue Shield at their Kentucky post office addresses, that mail will soon be returned as undeliverable.

Effective April 1, 2013, the forwarding order for mail sent to Anthem since October 2010 will expire. The company recently announced that they still receive some mail via the old address, including outdated Provider Adjustments forms that have not been updated by providers’ offices.

For updated forms, providers should visit www.anthem.com/home-providers.html , choose the state, and then choose “Provider Adjustment Forms” from the “Answers@Anthem” drop-down menu. Providers also should update their records with the correct addresses for claims and correspondence, included below:

  • CLAIMS: Anthem Blue Cross and Blue Shield, P.O. Box 105187, Atlanta, GA 30348
  • CORRESPONDENCE: Anthem Blue Cross and Blue Shield, P.O. Box 105557, Atlanta, GA 30348
  • FEP CLAIMS AND CORRESPONDENCE: Anthem Blue Cross and Blue Shield, P.O. Box 105557, Atlanta, GA 30348
  • GRIEVANCE AND APPEALS: Anthem Blue Cross and Blue Shield, P.O. Box 105568, Atlanta, GA 30348
